Help those with lung diseases "breathe easy”…
The work of The FACES Foundation continues to support its mission to acknowledge and promote professional excellence in the
education and care of patients with life threatening lung diseases. Please consider supporting the mission and work of The FACES
Foundation in one of the following ways:
A monetary donation
Participation in the Philip C. Lamka Memorial Golf Outing (held annually on the first Saturday in June)
Sponsorship of a PHIL Award:
If your life, or the life of a loved one, has been helped by the exemplary care of a respiratory therapist, tell us your story. FACES
will notify the hospital that you would like to sponsor an award. An individual or family may sponsor an award for an annual
contribution of $500.
If you work in a hospital or other healthcare setting and would like to bring the PHIL Award program to your workplace, please
contact us for turnkey materials. The PHIL Award will bring with it recognition and honor, and assist you in retaining a quality staff
of respiratory therapists.
Please contact us if support of the many faces of lung disease fits into your corporate giving plans.
For more information on any of these programs, please contact Sharman Lamka at 877-505-2075 or email@example.com.
Please use this form to make your donation.
Credit cards may be sent via email or print the form and send with a check

In time of crisis we're there to see
That everyone's breathing is finally at ease.
We strive to help when it's not the best
To make sure we give them a little time to rest.
A parent or family member watching can be tough.
We know it's not easy and can be really rough.
Just hang in there for we'll help get them there
Before having trouble breathing is too much to bear.
With the help of our machines and the treatments we give
Let's find a solution to better the life you live.
So, thank you for being our patient and may
Your RT help you have a better day!
